| HEALTH GUARANTEE/PURCHASE CONTRACT |
We want our puppies to be healthy and happy companions. We want the best start for our puppies. We believe a good vaccination program is essential. We vaccinate our puppies at 6 and 8 weeks of age with Galaxy DA2PPv for protection against Canine Distemper, Adenovirus Type 2, Hepatitis, Parainfluenza, and Parvo. We also start worming our puppies at 4 weeks old and worm them every week from then on with Pyrantel. We feed our puppies Eukanuba Small Breed dry puppy food. The puppy will come with a health certificate signed by the vet if it is to be shipped, and a current shot record.
The seller does not guarantee against Hypoglycemia, Coccidious, Kennel Cough, Parasites or other stress or environmental related conditions. The seller guarantees the puppy for 1 year from date of purchase for life threatening genetic disorders. The seller requires a written statement from the vet detailing the condition. If the puppy is found to have a life threatening genetic disorder, the seller would replace the puppy with one of equal value (when available) upon the return of the puppy and the puppy registration papers at the buyers expense. The Buyer may choose to keep the puppy, thus assuming all future medical costs. The seller will not be responsible for any veterinarian costs incurred by the buyer. This puppy is guaranteed to be in good health at the time of sale. Buyer agrees to take puppy to a licensed vet within 48 hours of leaving sellers possession for an examination to ensure puppy is not sick or injured at the time of purchase or delivery. Seller is to be sent a copy of the Veterinarians examination (Not a copy of the bill) within 7 days.. If Buyer fails to take puppy to a licensed vet within the 48 hours time limit and fails to send examination report to seller this guarantee is void and null. We make no guarantees on size, height, weight, conformation, disposition or breeding ability as adults. No Cash refunds will be made, and Buyer is responsible for all Veterinarian bills. The Buyer agrees to maintain preventative care of the puppy such as vaccinations, proper nutrition, etc. The Buyer agrees that should it become necessary to sell or otherwise place this puppy, the Buyer will notify the Seller before taking such action so that the puppy may, at the Seller's option, be repurchased by the Seller. This agreement is made to insure the well being and protection of this puppy. If any part of this Agreement is breached, the Seller is released from any futher obligation hereunder.
